Make an appointment to apply for a US Passport using Form DS-11 

Appointment hours are: Monday-Thursday 10 am-7pm 
& Friday and Saturday 10 am-3 pm

When requesting an appointment, please allow at least 24 hours for response. If urgent, please call (708) 343-1847 and ask to speak with Passport Manager. 

Before arriving, please have your appointment confirmation email available.

Is this a renewal for a minor child?

The library does not process DS-82 adult renewal forms. Most passports can be renewed by mail. The renewal application can be filled out and mailed by the applicant. Detailed instructions are included on the DS-82 renewal form.
