The library has limited notary services available, for free, by appointment only. Please review our Policy.

Before filling out the form below, read the following information:
1) Please do not follow-up this form with a phone call, email, or visit. We will contact you within 2 business days in regards to your request.
2) Per our Policy, you must have a library card to have a document notarized (with the exception of Passport form DS-3053 or DS-11).
3) All notarized documents must be in black ink.
4) Per our Policy, we do not notarize documents concerning property (ie Title Transfers, Quitclaim Deeds, or Rental Agreements), estates or estate planning, documents concerning livelihood (ie Power of Attorney), or child custody. 
5) The library does not provide legal advice or counsel. 
6) Signatures must be done in front of the notary and in-person.
7) The State of Illinois allows any notary to refuse a notarization without explanation.
8)  Dates and times for appointment requests must be made within library hours. Keep in mind, the library does not schedule notarizations within a half hour after opening or half hour until closing.
